Hospital stock accuracy starts with the movement ledger
For Stores manager / pharmacy operations lead · Understand the problem
TL;DR — what you’ll learn
- Reconcile stock by item, unit, location and status before using the total for decisions.
- Distinguish physical custody from stock authorised for issue.
- A transfer leaves one location before it is acknowledged at another; preserve that interval.
- Investigate missing events and unit errors before approving adjustments.
The central store and ward each show a plausible stock balance, yet a hospital-wide report cannot explain where ten boxes went. Someone proposes a stock adjustment so the dashboard agrees with the physical count.
Start hospital inventory management with the movement ledger. Reconstruct opening stock, receipts, issues, returns and authorised adjustments for a defined period. A total becomes useful when the team can explain the events that produced it.
Reconcile a defined stock boundary
In this illustrative central-store example, every movement uses individual units of the same item. The period and location remain fixed.

| Movement | Units | Running physical balance |
|---|---|---|
| Opening stock | 200 | 200 |
| Received | +80 | 280 |
| Issued out | −90 | 190 |
| Physically returned | +10 | 200 |
The closing physical balance is 200 units. In this synthetic state model, 20 units are restricted and all ten returns await review, with no overlap; 170 units remain in the example’s eligible category. The model demonstrates subtraction only. Actual release and handling criteria must be supplied by the responsible pharmacy or quality owner.
An issue to a ward also does not prove consumption by a patient. It changes custody within the hospital. If the hospital-wide ledger treats that transfer as final consumption, its stock and costing reports will answer different questions without saying so.
Keep transfers visible while they are incomplete
A transfer needs a source event, an in-transit state and a destination acknowledgement. The store might dispatch ten units while the ward acknowledges eight. Do not force either team to record ten simply to close the task. Hold the two-unit difference for investigation with both records attached.

This design distinguishes a delayed acknowledgement from a confirmed loss. Staff can follow the open handover without making an unsupported accounting or stock decision.
Look for the error before adjusting the quantity
For the ten-unit transfer, a reconciled network view might show zero of those units at the source, eight at the destination and two still in the unresolved transfer state. The network still accounts for ten units of custody; it has not established that ten are available at the destination. If a report sums source issues as consumption and also counts destination stock, it mixes events and balances. Keep the report's boundary explicit before interpreting the result.
This ledger explains where quantities came from. Expiry-action management uses that evidence to choose a reviewed action; it should not re-create a second stock ledger with different balances. Supplier-owned implant stock needs the additional ownership and settlement distinctions in the consignment guide.
Check posting cutoffs, missing returns and unit conversions. A physical count at 16:00 cannot be compared fairly with a ledger that includes a 16:30 delivery. A box-to-unit conversion should come from the approved item record and its applicable version, not a free-text guess.
Ask the vendor to demonstrate separate event and entry times, backdated entries and linked corrections in the intended configuration. Where those records are unavailable, document the reconciliation limitation and agree an alternative before relying on the resulting balance.
Bahmni's stock-management description includes moving and monitoring stock. In an evaluation, ask the vendor to demonstrate the underlying event history and exception states, not just the closing quantity.
Choose a practical control method
A small provider with limited movement may begin with a controlled ledger and explicit sign-off. A busy pharmacy or network may need scanning and system-enforced identifiers. Scanning reduces some typing; it does not prove that the correct item, batch or location was scanned.
Use one item category and two locations for an initial comparison. Include a partial transfer, a returned item held for review, a repeated scan and an entry after the count cutoff. An adjustment should require an authorised explanation linked to the investigated difference. Never make the integration create a balancing movement merely to make totals match.
Measure unresolved movement count, age and correction effort alongside stock variance. Keep the original discrepancy visible after resolution so the team can identify recurring causes.
The ledger is the explanation behind the stock number. Once custody, status and timing are explicit, an inventory report can support a decision without concealing the unfinished work underneath it.
Next actions
- Reconcile one item in one unit and location over a stated period.
- Follow its transfers and returns, including restricted and unacknowledged quantities.
- Agree correction authority and then evaluate which recording steps need system support.
Continue your evaluation
Use the expiry-action guide to decide how a reconciled stock ledger should support reviewed batch-level decisions.

The scenarios and numbers are illustrative. Confirm these provider-specific inputs before implementation:
- Pharmacy/stores lead and quality owner: Approved stock-state definitions, return review and release authority; validate that 170 units is only an illustrative eligible balance.
- Inventory system owner and stores controller: Current configuration and test of transfer discrepancies, count cutoff, duplicate scan and approved adjustment.
