Reconcile consignment implants before approving the supplier bill

TL;DR — what you’ll learn

  • Track ownership, physical custody, authorised use evidence and settlement separately.
  • Reconcile every unit or applicable identifier through use, return or an unresolved state.
  • Do not infer use from a missing item or payment from a physical receipt.
  • Pilot one supplier agreement with clinical-record, stores and finance owners involved.

Ten supplier-owned units arrive for a procedure-related consignment. Two have authorised use records, seven are acknowledged as returned and one cannot yet be explained. The supplier bills three. A total-quantity match can conceal the very unit that needs investigation.

The synthetic consignment model separates supplier ownership, hospital custody, recorded use, return evidence and proposed settlement. It assumes no universal liability trigger. The agreement and authorised clinical/finance owners must supply the meanings and commercial treatment before implementation.

Account for all ten units

This is an illustrative record-reconciliation case, not a clinical implant-use procedure. Use synthetic identifiers C-01 to C-10.

The reconciliation is 10 = 2 + 7 + 1. It does not prove the unresolved unit was used, lost or billable. Keep it open until the responsible teams establish the facts. Nor does the table alone authorise payment for the two recorded uses; commercial validation remains necessary.

The proposed reconciliation reads an authorised use-record reference; it does not create or infer a clinical use event. Have the provider’s clinical record owner define the acceptable source and identifier granularity for the actual product before testing any connection.

Build records around identity and custody

For the bounded pilot, capture supplier, agreement version, item identity, applicable serial or lot references, site, custody events, authorised use-record reference, return status and invoice line. The correct identifier granularity depends on the product and applicable requirements and needs specialist confirmation.

Use synthetic references for the initial reconciliation demonstration. Before introducing real data, have clinical-record and privacy owners decide the minimum references visible to procurement and suppliers. The guide does not establish a right to share patient or clinical details.

Compare a stock extension with a separate register

An existing HIS or ERP may support ownership and consignment states. Verify this rather than forcing supplier-owned units into ordinary purchased stock. A separate controlled register may be an interim option when volume is small, but reconciliation and permissions still need an owner.

Bahmni's billing and accounting documentation is a reference for financial-system boundaries; it does not establish consignment accounting treatment or implant traceability requirements. Those remain decisions for the contracted parties and qualified provider owners.

Returned, opened but unused and physically missing items may have different agreed treatments. Do not collapse them into a single “consumed” code to simplify matching.

Prove the pilot before changing settlement

Start with one site, supplier and agreement. Use read-only reconciliation first. The process owner reviews proposed states; the integration does not alter clinical use records or approve payment. Confirm supported source exports, identifier mapping, audit history and downstream adjustment methods with the vendors.

Test a missing identifier, conflicting use and return records, a partial return acknowledgement, an invoice repeated with a new scan and a correction after finance review. If evidence conflicts, retain both source references and stop the automated recommendation. A manual resolution must record who decided and why.

Acceptance requires every received unit to have a supported state or an explicit unresolved case. Report unresolved age, missing evidence and the effort required to reconcile supplier statements. Count duplicates prevented in the test set, but do not promise that software eliminates all loss or disputes.

Resolve the eleventh-hour discovery without rewriting the case

Suppose the unresolved C-10 is located in an authorised holding area. The current reconciliation becomes two recorded uses, seven acknowledged returns and one unit in confirmed hospital custody. It does not become an eighth completed return until the applicable return handover is evidenced. Its commercial treatment remains subject to the agreement and authorised review.

Retain the earlier unresolved state and the evidence that closed the location inquiry. The stores owner confirms the custody event; the clinical recorder remains responsible for any use evidence; finance reviews the effect on the supplier bill. No participant should need to invent another team's event to complete their own task.

Keep the first pilot read-only: it proposes reconciliation states and produces an exception list. Stop recommendations for any unit whose identity or evidence conflicts. This preserves a useful outcome even before settlement automation is justified. The general stock ledger explains quantities; this additional ownership-and-evidence model explains why custody alone cannot settle a consignment bill.

Next actions

  1. Confirm the agreement's ownership and settlement triggers with finance and procurement.
  2. Have clinical-record and stores owners approve the evidence and identifier model.
  3. Reconcile a synthetic ten-unit case, including the unresolved item, before proposing invoice automation.

Discuss this workflow

Discuss a consignment-reconciliation pilot for one supplier agreement, with clinical-record, stores and finance evidence kept distinct.

Describe the operation using business information only. Please leave out patient details.

Applying this guide

The scenarios and numbers are illustrative. Confirm these provider-specific inputs before implementation:

  • Procurement, finance controller and legal adviser: Current consignment agreement and approved accounting treatment; validate that an unresolved unit is not presumed billable.
  • Authorised clinical recorder, OT lead and quality owner: Current product/category requirements and approved recording SOP; no clinical use or substitution advice.
  • Privacy lead and medical records owner: Approved access/data-flow map and reference-link design with only necessary information exposed.
