Track rehabilitation packages without treating attendance as clinical progress

TL;DR — what you’ll learn

  • Keep the clinical plan, appointment record and commercial session entitlement distinct.
  • An attended session is not a measure of recovery or treatment effectiveness.
  • Apply the approved cancellation policy explicitly rather than letting a status consume entitlement by accident.
  • Preserve corrections and reconcile balances when a package pauses or a provider changes.

A rehabilitation centre sells a ten-session package. Six sessions have been attended, one booking was cancelled and three future bookings remain. The dashboard displays “70% complete” because it counts the cancelled slot, while the therapist's assessment describes the care plan separately.

A rehabilitation package record should explain attendance and commercial entitlement without translating either into clinical progress. Link each appointment to its authorised plan reference and package allocation. Then apply the approved commercial rule to cancellations, pauses and adjustments, leaving clinical assessment with the responsible professional.

Use three records for three different questions

The clinical plan answers what care is authorised and how it is assessed. The appointment record answers what was scheduled and what happened to that booking. The entitlement ledger answers which commercial units were purchased, consumed, restored or adjusted under the agreed terms.

The clinical plan and its meaning remain with the responsible clinician. The proposed package ledger records commercial units and links only to the plan references authorised for that purpose. Ask clinical and privacy owners to approve those references and disclosures before use. Six attended sessions or four remaining entitlements establish no measure of recovery or requirement for future treatment.

Bahmni describes billing and accounting functions. It does not supply a rehabilitation entitlement policy. Ask the selected version to demonstrate the approved attendance-to-charge relationship, repeated calendar events and authorised reversals with an exportable history. An unexplained balance or unsupported correction remains an evaluation gap.

Reconcile ten entitlements under two possible policies

Start with ten purchased session units and six attended sessions. There is one cancelled appointment and three pending appointments. If the approved policy restores a cancelled booking without consumption, six units have been used and four remain: three may be reserved for the pending appointments and one unreserved.

If a different approved policy consumes a unit for this cancellation, seven units have been used or charged and three remain. The commercial outcome changes, but actual attendance remains six in both cases.

The two cancellation cases are arithmetic alternatives. Before configuring either, obtain the applicable agreement and the commercial/legal owner’s approved disclosure, consumption and adjustment rules. Keep expiry, refund and transfer treatment outside the example unless separately established. The article does not endorse charging for cancellation or determine a person’s entitlement under an actual agreement.

Compare package and per-visit records

Per-visit billing can connect each authorised charge directly to its appointment and payment record. Package billing adds a balance that spans visits. That balance needs an identifiable source purchase, an agreed unit definition and a complete movement history.

A package reservation should not count as consumption unless the approved policy explicitly defines that event. If an appointment moves to another date, carry the reservation or release and recreate it with a link. Repeated calendar messages must not consume a second unit.

Make pauses and corrections visible

A pause can affect scheduling, package terms or both. Store the requested interval, authorisation and commercial effect explicitly. The system should not quietly extend an expiry or recognise a refund because an appointment was cancelled. A provider change similarly requires a reviewed handover and whatever commercial transfer is permitted, not a copied completion percentage.

Test a session wrongly marked attended, then corrected to cancelled. The history should retain the original event and an authorised reversal or adjustment. Ask for a report that separately reproduces the six attended sessions, the entitlement movements and the future bookings. Exporting just the latest balance is insufficient to investigate a discrepancy.

The centre may track attendance rates for operational planning, but the numerator and eligible appointment population should be explicit. A ten-unit package is not automatically the denominator for a clinical performance measure. Keeping those concepts separate gives reception, finance and clinicians a record they can each interpret correctly.

Next actions

  1. Define the commercial session unit and approved cancellation, pause and adjustment rules.
  2. Reconstruct both ten-session examples while keeping attendance fixed at six.
  3. Test a corrected attendance event and a provider handover, then reconcile the entitlement history.

Applying this guide

The scenarios and numbers are illustrative. Confirm these provider-specific inputs before implementation:

  • Authorised rehabilitation professional and clinical records owner: Approved care-plan, assessment and access process; no outcome scoring from attendance.
  • Provider commercial owner, finance and legal reviewer: Applicable patient agreement, approved policy and adjustment authority.
  • Practice system owner and finance implementation lead: Demonstrations of repeated events, reversals, pauses and reproducible balance exports.
