Billing and revenue cycle

Reconcile TPA settlements without hiding deductions

TL;DR — what you’ll learn

  • Allocate a bulk receipt to claims without treating cash received as full settlement.
  • Record deductions, disputes and unexplained balances separately.
  • Keep proposed matches distinct from finance-approved allocations and adjustments.
  • Reconcile repeated remittance advice and partial payments before automating posting.

A hospital receives ₹90,000 against claims totalling ₹100,000. The remittance explains ₹6,000 of deductions, while ₹4,000 remains unclear. The reconciliation tool closes every claim because the payment file imported successfully.

TPA reconciliation software should preserve the difference between money received and the amount still needing a decision. A match can identify which claims a receipt relates to without authorising a deduction or write-off.

Allocate cash and retain the remainder

The following amounts are illustrative. Assume the remittance provides sufficiently clear references for finance to review this proposed allocation.

Allocate cash and retain the remainder
ClaimClaimed amountCash allocationStated deductionUnexplained balance
C-101₹60,000₹55,000₹3,000₹2,000
C-102₹40,000₹35,000₹3,000₹2,000
Total₹100,000₹90,000₹6,000₹4,000

The synthetic allocation is ₹55,000 + ₹35,000 = ₹90,000 cash, with ₹6,000 stated deductions and ₹4,000 unexplained. These labels preserve evidence for finance review. They do not establish accepted deductions, revenue recognition, tax treatment or write-off authority.

Match using references before interpreting descriptions

The pilot needs the bank receipt, remittance identity, payer, provider entity, claim reference, billed and submitted amounts, prior receipts, deduction codes, dispute references and adjustment history. Preserve the source line for every proposed allocation.

Use exact supported references first. Amount and date can help narrow candidates but should not overrule conflicting identifiers. An AI assistant may extract a remittance table or suggest a match from text. An ambiguous allocation stays in a reviewer queue with the original advice visible.

Hayati describes hospital-side payer workflow coordination. Use the actual payer’s advice and agreed interfaces to define a matching pilot. No remittance format, live connectivity or settlement term is assumed from that vendor description.

Separate the decisions in the workflow

The revenue-cycle team can investigate a claim reference or deduction. The payer desk can seek clarification. Finance approves allocations and accounting adjustments within its authority. The system should preserve a disputed deduction as disputed even when the receipt itself is fully allocated.

Do not use an automatic “balance to zero” action as the resolution path. A claim may legitimately remain open after some money arrives.

Pilot one payer and one entity

Begin with read-only proposals for one payer relationship and inspect them with finance. Ask HIS/accounting owners to demonstrate identifiers, prior allocations, correction semantics and restricted adjustment permissions. Keep posting disabled where those capabilities have not been established.

Include a later partial payment against C-101. The next allocation must consider the earlier ₹55,000 and current approved adjustments; it must not treat the original ₹60,000 as wholly unpaid again. Also test a remittance covering more than one entity and a payment with no usable advice. Cross-entity treatment requires finance validation.

Recover from repeated and corrected advice

A duplicate advice document is not new money. Identify the receipt and advice independently, retain versions and compare their effects. A revised deduction code may change the review task without changing the bank amount. If a posting response is lost, reconcile the destination reference before retrying.

During an outage, keep proposed allocations on hold and use the approved manual process. Capture manual postings and decisions before resuming automated work. Pausing the pilot must not erase disputes or reverse valid receipts.

Acceptance requires allocations never to exceed the actual receipt, replay to create no extra cash in the test set, and every remaining deduction or unexplained balance to have an owner. Measure unallocated cash, disputed deductions and unresolved claim balances separately, along with their age and correction effort.

Reclassify a balance without inventing another payment

Suppose a revised remittance later explains the remaining ₹4,000 as additional deductions. The existing ₹90,000 receipt and cash allocations do not change. The record now shows ₹10,000 of stated deductions awaiting the appropriate acceptance or dispute decisions, rather than ₹6,000 stated and ₹4,000 unexplained. This is a change in evidence classification; it is not new cash and does not authorise a write-off.

Keep the first advice, revised advice and reason for the new classification together. If an authorised reviewer disputes a deduction, preserve its value and case reference outside any automatic closure rule.

Finance should sign off the cash allocation and deduction decision separately. Compare opening claim balances, cash, approved adjustments and closing balances for each synthetic case; a matching bank total alone cannot show that individual claims are correct. The pilot succeeds when those relationships and the unresolved work remain explainable after revised information arrives.

Next actions

  1. Agree allocation, deduction and closure definitions with finance and the payer desk.
  2. Reconcile the synthetic ₹90,000 receipt without hiding the ₹10,000 remainder.
  3. Test partial payments, revised advice and replay before permitting controlled posting.

Discuss this workflow

Discuss a TPA settlement pilot for one payer and entity, from remittance evidence to reviewed allocation, deductions and unresolved claim balances.

Describe the operation using business information only. Please leave out patient details.

Applying this guide

The scenarios and numbers are illustrative. Confirm these provider-specific inputs before implementation:

  • Finance controller and qualified accounting/contract adviser: Approved payer terms and accounting policy; validate the ₹90,000 allocation without treating deductions as automatically accepted.
  • Payer desk and payer integration owner: Current payer documentation and representative synthetic remittance specification; no vendor outcome or universal connectivity claim.
  • HIS/ERP owner: Version-specific tests for partial payment, revised advice, duplicate document and lost posting response.
