Rehearse hospital downtime and the return to digital records

TL;DR — what you’ll learn

  • Maintain pre-allocated temporary downtime identifiers (TMP-UHID) and controlled paper forms so admissions, emergency orders and cash receipts continue safely when the HIS is unreachable.
  • Never declare an outage "Resolved" the moment the server pings green—system restoration is complete only after downtime paper records are reconciled and handback is authorised.
  • Require deterministic matching before transcription so temporary downtime registrations do not create permanent duplicate patient identities or double-billed charges.
  • Verify that clinical orders, pharmacy dispenses and diagnostic results captured on paper during the outage are linked to the authoritative encounter in chronological sequence.

During a 3-hour planned-plus-extended HIS database maintenance window (DWN-REC-2026-10), a 220-bed hospital operates on its continuity procedures across emergency, OPD registration, pharmacy and inpatient billing, generating 140 paper downtime transactions: upon system restoration, 112 transactions are transcribed and reconciled cleanly against existing UHIDs and active encounters; 12 emergency walk-in patients registered on temporary downtime numbers (TMP-DWN-01..12) are found during MRD check to have existing permanent UHIDs that must be linked rather than created as new duplicate patients; 10 stat pharmacy issues dispensed on paper slips were re-entered by the ward clerk *and* auto-imported from a delayed barcode buffer, risking double billing (₹18,400 duplicate charge exposure); and 6 paper cash receipts issued from the manual receipt book (MREC-801..806) were not yet posted to the shift cashier ledger when the night supervisor tried to close the shift. If IT announces "HIS Back Online — Normal Operations Resumed" without a structured handback gate, those 28 exceptions (12 + 10 + 6) corrupt permanent clinical and financial records.

For hospital continuity leads, CIOs, medical-records heads, nursing superintendents and finance controllers, the core question is what should happen to administrative and operational records created while the HIS is unavailable—and how to govern the return to digital records.

Separate continuity from clinical protocols

Hospital downtime readiness has two halves that are equally vital: *continuity during the outage* (giving reception, emergency, wards, labs, pharmacies and cash counters read-only local reference snapshots plus controlled, pre-numbered downtime forms and TMP-UHID blocks) and *reconciled recovery after restoration* (entering, matching and verifying every temporary event in chronological order before normal automated billing and closes resume).

The CERT-In cybersecurity directions index, Bahmni feature list and Bahmni registration documentation provide resilience/incident context and first-party product examples of registration and module boundaries (context only; clinical emergency triage during outages, medication safety protocols and financial receipt custody must be governed by the hospital’s medical director, nursing/pharmacy leads and finance controller).

In the synthetic 140-transaction DWN-REC-2026-10 post-outage recovery ledger, every paper downtime record is tracked from serial-number log to verified HIS reconciliation:

All 140 downtime transactions (112 + 12 + 10 + 6 = 140) are reconciled against pre-numbered form ranges and temporary ID registers: the 12 temporary emergency IDs are linked to permanent UHIDs without duplication, the 10 buffered pharmacy issues (₹18,400) are deduplicated, and all 6 manual cash receipts (MREC-801..806) are posted before cashier sign-off.

Compare read-only access and approved paper capture

A critical design requirement for post-downtime transcription is preserving *two timestamps* on every back-entered record: actual_event_time (when the patient was admitted, the sample was drawn or the cash was received on paper at 02:15) and system_recorded_time (when the clerk transcribed the form into the restored HIS at 05:10, tagged Downtime Back-Entry). If the HIS uses only system_recorded_time, an emergency admission at 02:15 appears to have occurred *after* a 04:50 lab result, breaking clinical timelines and TPA claim audits.

Handle duplicate identities and delayed charges

Outage recoveries fail when wards use unnumbered scratch paper (making it impossible to know whether 134 or 140 events occurred during the 3 hours), when registration clerks create brand-new permanent UHIDs for returning patients (TMP-DWN-01..12), or when interface queues flush delayed barcode events on top of manually keyed downtime entries (₹18,400 duplicate pharmacy charges in DWN-REC-2026-10).

In the DWN-REC-2026-10 workflow above, decision gate D blocks normal discharge billing and shift-close sign-off until every serialised downtime form and TMP-UHID is accounted for. Discrepancies route to E, receive MRD, pharmacy or finance resolution at F, and re-verify at C before formal handback at G.

Rehearse restored-system reconciliation and authorised handback.

Rehearse a 2-hour downtime and recovery drill (DWN-REC-2026-10) at least twice a year covering emergency registration, inpatient medication issue, lab orders and cash collection. Verify that the HIS supports dual timestamps (actual_event_time + system_recorded_time) and that all 140 synthetic drill forms reconcile with zero duplicate UHIDs or double charges.

Software and controlled binders provide read-only snapshots, temporary ID ranges, dual-timestamp back-entry and serial reconciliation; clinical care priorities during outages, identity merge approval and financial shift sign-off remain with clinical leaders, the MRD head and the finance controller.

Next actions

  1. Replace unnumbered downtime forms with pre-numbered serialised packs and pre-allocated TMP-UHID blocks (DWN-REC-2026-10) in every critical unit.
  2. Configure your HIS back-entry screen to capture both the original paper event timestamp and the transcription timestamp (Downtime Back-Entry).
  3. Add a mandatory post-outage reconciliation gate requiring MRD, pharmacy and cashier serial-number sign-off before discharges or shift closes resume.

Applying this guide

The scenarios and numbers are illustrative. Confirm these provider-specific inputs before implementation:

  • Hospital continuity lead, nursing superintendent and medical-records (MRD) head: Approved downtime binder stock, `TMP-UHID` allocation register and post-outage MPI linkage SOP.
  • Finance controller, billing head and pharmacy manager: Verified `DWN-REC-2026-10` reconciliation catching the `₹18,400` duplicate buffer charges and balancing all manual receipts (`MREC-801..806`).
  • CIO and HIS / integration administrator: Read-only local census snapshot mechanism, dual-timestamp back-entry support and controlled interface-queue replay.
