Build cash-flow visibility from receivables and committed purchases

TL;DR — what you’ll learn

  • Separate accrued billing revenue from expected cash collections in rolling hospital treasury forecasts.
  • Group inflows and outflows by certainty tier—confirmed bank balances, scheduled TPA/corporate settlements, and committed purchase/payroll obligations.
  • Model payer collection delays explicitly alongside non-deferrable payroll, statutory and critical supplier due dates.
  • Preserve actual bank closing balances and prior forecast snapshots so variance explanations remain auditable.

A 120-bed hospital reports strong monthly billed revenue, yet faces a short-term working-capital squeeze in Week 3 of its 13-week rolling cash view (CFW-13W-08). An expected ₹20.00 lakh bulk settlement from two Third-Party Administrators (TPAs) shifts outward by 14 days while pending query responses are reviewed, whereas ₹42.00 lakh in staff payroll, statutory tax remittances and critical reagent supplier payments remain strictly due in Week 3. A finance report that equates billed inpatient revenue with available cash gives leadership no warning of the ₹22.00 lakh timing gap.

For hospital CFOs, controllers and promoters, the essential question is how to build a rolling cash-flow forecast from actual receivable ageing and committed purchases without hiding collection timing risks.

Separate booked revenue from expected cash

Hospital revenue and hospital cash move on different clocks. Outpatient cash and UPI collections settle within 24–48 hours, whereas credit bills under insurance TPAs, corporate tie-ups and government schemes pass through discharge documentation, submission, query adjudication, deduction review and batch remittance before reaching the bank. Meanwhile, outflows follow rigid calendars: monthly salary runs, PF/ESI/TDS statutory dates, loan EMIs, equipment lease instalments and critical pharmacy/implant vendor credit limits.

Bahmni billing and accounting and Hayati TPA billing illustrate the operational boundary between hospital billing/accounting records and external TPA claim workflows (reference context only; treasury management, banking covenants and tax remittance schedules require review by the hospital’s finance controller and treasury/tax advisers).

In the illustrative CFW-13W-08 Week-3 cash-flow bridge (all figures in ₹ Lakh), base-case and delayed-payer scenarios are compared against committed obligations:

In the base scenario, Week 3 closes at ₹35.00L + ₹28.00L + ₹20.00L - ₹30.00L - ₹24.00L = ₹29.00L (above the hospital’s illustrative ₹15.00 lakh minimum operating buffer). When the ₹20.00 lakh TPA remittance shifts by two weeks, projected closing cash drops to ₹9.00L (₹6.00L below the ₹15.00 lakh buffer), triggering an early working-capital review.

Map commitment versus payment

A useful cash-flow tool does not treat every open invoice in the 30-day ageing bucket as equally collectible next week. Instead, it separates claims that have received a formal payer settlement advice from claims still in "Submitted" or "Query Pending" status, and pairs them with committed purchase orders (not just posted supplier invoices) so large upcoming capital or reagent deliveries appear before the invoice arrives.

Compare base and delayed-payer scenarios

Three data gaps regularly distort hospital cash forecasts: bulk TPA bank transfers that sit in an unallocated receipt suspense account because remittance advice files have not been matched, emergency implant or drug purchases ordered outside standard PO workflows, and patient refunds or consultant payouts approved mid-week without updating the outflow schedule. Additionally, overwriting last week’s forecast tab with this week’s actuals destroys the team’s ability to learn which payer buckets consistently slip.

In the CFW-13W-08 workflow above, decision gate D blocks publication of the weekly forecast until bank balances reconcile and any buffer breach (₹9.00L vs ₹15.00L threshold) has an owner-reviewed action note (E and F) before CFO sign-off (G).

Update forecasts without overwriting actuals

Pilot the 13-week rolling cash model (CFW-13W-08) over four consecutive weekly cycles alongside existing treasury spreadsheets. Require each week’s opening balance to tie exactly to bank statements, track forecast-to-actual variance by inflow/outflow category, and test the delayed-payer scenario (-₹20.00L) so finance can sequence non-clinical discretionary spend without touching statutory dues or critical medical supplies.

Cash-flow planning software organises receivable, payable and bank data under transparent assumptions; credit facility decisions, statutory remittance compliance and vendor payment approvals remain with the CFO and authorised finance signatories.

Next actions

  1. Split your accounts-receivable forecast feed by claim stage (adjudicated settlement advice received vs submitted/queried) rather than using flat ageing percentages.
  2. Run the Week-3 CFW-13W-08 base (₹29.00L) and delayed-TPA (₹9.00L) scenarios to establish an explicit operating cash buffer trigger.
  3. Lock each weekly forecast snapshot before rolling forward so Friday treasury reviews compare actual collections against the prior week’s committed assumptions.

Applying this guide

The scenarios and numbers are illustrative. Confirm these provider-specific inputs before implementation:

  • CFO, financial controller and treasury lead: Approved 13-week cash-flow structure, certainty-tier definitions and minimum operating buffer policy (`CFW-13W-08`).
  • Accounts receivable / TPA desk lead and accounts payable manager: Signed weekly collection assumptions and three-way matched supplier maturity schedules tied to subledger exports.
  • ERP / finance systems administrator: Automated snapshot archiving preserving prior forecast versions alongside actual bank reconciliation imports.
